Immunizations 

Wisconsin Immunization Law (Wis. Stat.§ 252.04) sets minimum required immunizations for children attending school according to each age/grade level.  Schools are required to maintain current written documentation of all student's immunization status and ensure compliance with the law. Parents must submit their child's immunization record to the school on or before the first day of school and when additional vaccines are given.  

The exception to receiving the vaccines is to sign a waiver for one of three options health, religious, or personal conviction. Failure to comply with the law may result in legal action from the District Attorney.

If your child needs immunizations contact your physician's office to schedule an appointment. Immunizations are also available free or a reduced cost at your local health department.  

NEW IMMUNIZATION REQUIREMENTS for the 25 SCHOOL YEAR 

MenACWY (Meningococcal Vaccine)  

  • Students entering 7th grade will be required to provide the date of receiving one MenACWY containing vaccine. 

  • Students entering 12th grade will need to provide dates for two doses of MenACWY containing, unless the first dose was after their 16th birthday, then these 12th graders only need one dose.

  • The MenACWY requirement will be phased in for 12th graders. Meaning in the 2025/2026 school year those students graduating in 2026 (11th graders this current school year) will be assessed and need to be compliant via vaccination or having a signed waiver.

Varicella Vaccine

  • If you have a child entering our school district for the first time in the 2024/25 school year and your child does not need the varicella vaccine because they had chicken pox disease you will need to have a health care provider (MD, PA, or NP) sign the Immunization and Waiver Form verifying your child had chickenpox.

As your child received additional immunizations, you are required to provide the school with an updated immunization record. Please note in order for your child to be compliant with school entry requirements for immunization they either need to meet the specific immunization dose requirements or have a signed waiver on file with the school. The signed waiver can have a personal conviction or religious waiver for the specific vaccination(s), or have a medical waiver signed by a healthcare practitioner.

Protect Your Teen From Harmful Disease

The following immunizations are not required but are recommended for teens by the American Academy of Pediatrics and the Center for Disease Control.

Human papillomavirus (HPV) Vaccine  -  Protects men and  women from HPV infection/genital warts and cancer caused by HPV.  Protection is most effective if given before a person is sexually active but can be given at any time up to your early twenties.Two vaccines are recommended beginning at age 11.
