Important Information About Registering Your Child
Current Cudahy residents must register their children for the 2024-2025 kindergarten program, unless they already attend our CSD 4K program. If that's the case, you will receive a confirmation letter from your school.
Children must be five years old by September 1, 2024 to register for kindergarten. Registration is done online, by following the steps below. You must also provide your child's birth certificate, immunization records, and proof of residency.
Proof of Residency documentation could be a utility bill, lease agreement, mortgage documents, etc. Documents for proof of date of birth and residency need to be brought to our district office if they are not attached to the New Student Application.

Early Entrance to Kindergarten: In accordance with Cudahy Board of Education Policy 5112 Entrance Age, the District shall prescribe procedures, conditions, and standards for early admission to kindergarten. Please contact the building principal or the Student Service Director at 414-294-7421 for more information on Entrance Age policy and procedures.

How to Register for 5-Year-Old Kindergarten
Step One
DETERMINE HOME SCHOOL BOUNDARIES
Using the District Boundary Map verify that you do live in the boundaries serviced by the School District of Cudahy.
If you are not sure which elementary school is your neighborhood school of attendance, please call the Administration Building 294.7400)
Step Two
STEP 2: COMPLETE THE NEW STUDENT ONLINE ENROLLMENT PROCESS (NSOE)
Request a Username/Password
Username and Password will be emailed to you instantly
Use the new username (email address) and random password and begin completing the enrollment forms
Basic demographic information on family and each student
Medical Alert
Special Dietary Needs
Additional Parental Sign-offs
Student Handbooks
Directory information
Photo Release
Immunization Records Access (only if from Wisconsin)
SUBMIT
Your information will be reviewed by district personnel and once verified you will receive an email with further instructions on how to complete the enrollment.
You should receive this within 2 business days... if not please call 414.294.7400 and ask about yours.
Step Three
STEP 3: VISIT YOUR SCHOOL/DISTRICT OFFICE TO VERIFY RESIDENCY & AGE
This step includes visiting the Registrar at the Administration Building (2915 E. Ramsey Avenue) to provide Proof of Residency, Proof of Age and Immunizations (non WI residents).
